Information in the following form will help us determine if your organization can apply for Oklahoma Arts Council grant funding. New applicants and organizations that have not applied for Oklahoma Arts Council funding within the past three years must complete the form and submit all required documents as instructed below. If eligibility is approved, login credentials will be issued to your Grant Contact and your Authorizing Official. Your Grant Contact can then log in to complete a grant application. Additional Required Documents for Nonprofit Organizations
Nonprofit organizations must be tax-exempt 501(c)(3) organizations incorporated in the state of Oklahoma in order to be eligible for grant funding. Nonprofit organizations must submit copies of the following before eligibility can be determined. Forms can be uploaded in the Grant System Login Request Form below:
501 (c)(3) Determination Letter
Full IRS Form 990 or most recent financial statement
Oklahoma Certificate of Incorporation with Articles of Incorporation
