The online form below will help us determine if your organization is eligible for Oklahoma Arts Council grant funding. Please complete the form and submit all required documents. If eligibility is approved, logins and passwords will be sent to your Grant Contact and your Authorizing Official. Your Grant Contact can then log in to complete a grant application.
After you have submitted the online form, we will send the following to your Grant Contact. These must be completed and returned to the Oklahoma Arts Council before login permissions will be provided:
- An Approval Form (signed by your Authorizing Official)
- A Vendor Payee Form
Nonprofit organizations must also submit copies of the following before login permissions will be provided:
- 501 (c3) Determination Letter
- First page of your most recently submitted IRS Form 990
- Oklahoma Certificate of Incorporation
- Articles of Incorporation
Please note we cannot fund individuals, religious organizations, state agencies and for-profit entities
