Each application will be reviewed on the overall strength of the organization
as well as specific programming component criteria. (See Programming
Components)
Sustainability (0-20)
- Strength of leadership within organization structure
- Fiscal management of organization
Artistic Quality (0-20)
- Clearly stated and appropriate artistic goals that directly relate to the organization’s mission and strategic plan.
Outreach & Diversity (0-20)
- Outreach to underserved constituencies
- Breadth of diversity within the organization’s structure and programming
Assessment (0-20)
- Effective self-assessment methods
Support Material (0-20) (See Programming Components for Additional Support Material requirements)
- Demographic Profile Form of the community served (information is available from the U.S. census website at: http://censtats.census.gov/pub/Profiles.shtml)
- Resumes of Key Administrative Personnel within the organization. (Limit one page per person.)
- (For 501 (c)(3) organizations only) Board list with names and addresses and community affiliations/profession. Minority representation on the board should be indicated with an asterisk (*).
- (For 501 (c)(3) organizations only) IRS form 990 for most recently completed accounting year.
