Application Instructions

1. MAIL OR HAND DELIVER 3 copies of the completed application and supporting materials to the Oklahoma Arts Council (address below) either postmarked December 15, 2007 or hand delivered by 4:30pm on that day.

2. Application should be typed or filled out on a computer using the latest version of the free software Acrobat Reader.
3. Support materials that must accompany this application include:
• 1 copy of a Video DVD of a recent (no more than 1 yr. old) live performance representative of the programming being proposed. 5-10 minutes in length
  
• 3 copies of Artist’s/Group’s Media Kit

NOTE: All support materials submitted with the application will become property of the Oklahoma Arts Council. No support materials will be returned. Do NOT submit original/irreplaceable materials. Adobe Acrobat required.
